What is an orthodontic retainer?
A custom dental device worn after orthodontic treatment (braces or aligners) to maintain teeth in their corrected positions and prevent them from shifting back.
After braces or clear aligners come off, teeth naturally want to drift back toward their original positions. A retainer prevents this movement by holding teeth in place while the bone and soft tissue around them stabilize. Orthodontists typically recommend lifelong retainer wear, even after treatment ends, though the schedule may change from nightly wear to a few nights per week.
Two main types of retainers are used in orthodontics:
- Fixed retainers: Thin wires bonded permanently to the back surface of front teeth. They stay in place 24/7 and require no removal, but need careful flossing and brushing around the wire.
- Removable retainers: Custom-made plastic or acrylic devices worn at night or during the day. Common designs include Hawley retainers (metal wire and acrylic frame) and clear plastic retainers similar to aligners. These are easier to clean but depend on patient compliance.
The choice between fixed and removable retainers depends on your specific case, bite pattern, and lifestyle. Many orthodontics providers in Ghana recommend a combination approach, using a fixed retainer on lower front teeth paired with a removable retainer for broader protection. Without retainer wear, teeth can shift within months or years, sometimes undoing years of treatment gains.
